Middle Sports Physical Forms

We have 3 sports seasons: 
  • Fall (co-ed soccer, girls volleyball and co-ed cross country)
  • Winter (girls and boys) basketball
  • Spring (baseball and softball)
 
Students in grades 6-8 are eligible to participate in sports; 5th graders may be eligible if participant numbers permit.  Please note that the sports physical forms have changed for the 2024 Fall season.
Due dates for sports physicals:
  • Fall season physicals are due to the health office on August 19, 2024 -Start Dates TBD
  • Winter season physicals are due to the health office on November 11, 2024- Start Dates TBD
  • Spring Season physicals are due to the health office on March 10, 2025- Start Dates TBD 
In order to participate, your athlete needs:

REQUIRED FORMS:

These forms are given to your child’s doctor/medical home and remain there:
  1. Health History - filled out by parent/guardian
  2. Physical Exam Form - filled out by medical care provider
These forms must be returned to the school nurse: 
  1. PPE Medical Eligibility Form - filled out by your child’s medical provider
  2. Health History - completed by parent/guardian every season unless the physical was dated less than 90 days ago
  3. Permission to Participate Form - completed by parent/guardian; must be submitted for every sport an athlete plays
  4. Opioid Sign Off Form.  Please read the Opioid Fact Sheet before signing. Submit once a year; covers all sports seasons.
  5. Sudden Cardiac Arrest Sign Off Form.  Please read the Sudden Cardiac Arrest in Athletes Sheet before signing. Submit once a year; covers all sports seasons.
  6. Sports Related Concussion Fact Sheet and Sign Off Form. Submit once a year; covers all sports seasons.
  7. Please read the Sports Eye Injury Information Sheet (there is no sign off form)
Additional Forms (if applicable):
 
  • If your athlete has an Epipen for a severe allergy or has asthma, they MUST have emergency medication forms on file AND emergency medication available in order to participate.
  • Athletes who have sustained a concussion or sports-related orthopedic injury in the past year must have a clearance on file in the health office. 
  • Athletes with cardiac issues must have a cardiac clearance on file in the health office.
