Requirements

1. Copy of Birth Certificate
2. Proof of Residence like utility bill, driver's license, tax bill, rental agreement
3. Copy of Up-to-date Immunizations
4. Copy of Most Recent Examination
5. Please preregister by selecting Online Registration Form
